The service relationship

Form Indiana is the service brand of Indy Registered Agent, an independent filing and registered agent service. We are not a government agency, law firm, or tax adviser. Ordering a service does not create an attorney-client relationship.

Your information and authority

You confirm that you are authorized to act for the business and that the information you supply is accurate. You authorize us to prepare and submit the selected filings using those details. We may request clarification, signatures, or additional documentation before proceeding. Do not submit an application for a business you are not authorized to represent.

Pricing and state fees

The review step shows the service fee, state filing allocation, optional services, and total charge. State fees are separate from our fees. Standard state payment surcharges are absorbed in our service fee. Unusual filings or additional work require a separate agreement before any additional charge.

Formation and filing outcomes

Payment begins application review. It does not form an entity or guarantee government acceptance, name availability, an EIN, or a processing deadline. Government agencies make approval decisions. You remain responsible for licenses, tax obligations, and requirements outside the purchased service.

Self-service documents

The operating agreement builder produces a self-service document from the answers you provide. It is not legal or tax advice, attorney drafting, or a government filing. You are responsible for reviewing the agreement, confirming that all members understand it, obtaining signatures, keeping the signed document with company records, and seeking professional review for complex ownership, tax, governance, investor, or industry-specific terms. A completed document order is generally nonrefundable once download access is provided, except as required by law.

Registered agent appointment

Our registered agent service begins when we accept the appointment and any required state change is effective. The registered office receives official legal and state correspondence within the purchased service scope. It is not a virtual office, general mailing address, or package receiving service. You must keep your contact information current and act promptly on delivered notices.

Renewals and cancellation

Standalone registered agent service renews annually at $49 per business until canceled through the billing portal. LLC formation includes one year of agent service with no automatic package renewal; continuing after that year requires a separate renewal choice. Business Entity Report and EIN assistance orders are one-time services. Canceling payment renewal does not dissolve a business or appoint a replacement agent. Arrange continuous agent coverage when required.

Refund requests

Contact us promptly to cancel an order. We refund our fee for work that has not started. Once work begins, contact us so we can review what has been completed and any appropriate refund. Government fees already paid and completed services are generally nonrefundable. If we cannot perform an ordered service, we will explain the issue and address any unearned fees. This does not limit rights available under applicable law.

Documents and responsibility

Keep copies of your business records and read notices promptly. Reminders are a convenience and do not replace your obligation to meet deadlines. A generated operating agreement may not suit complex ownership, tax, governance, investor, or legal circumstances.

Contact and changes

Use the contact form or support workspace for service questions, cancellation requests, and disputes. We will provide notice of material changes to ongoing service terms or pricing before they take effect. The terms accepted with an order are recorded with that order.
